Care transitions among women living with dementia
Synopsis
Unplanned moves into hospital or residential aged care can be stressful for people with dementia and their families. Planned transitions to hospital and to residential care may ease pressure on emergency services, and assist in maintaining quality of life for people with dementia and their families. This project will investigate planned and unplanned transitions to residential aged care and hospital and the factors associated with these.
